If you have received a survey from NRC Health on behalf of your healthcare provider or other customer of NRC Health, this Notice does not apply to information collected as part of such survey.  If you have questions regarding the privacy of information provided in sch survey, or if you need to access, amend, or delete any information we may have collected from you on behalf of our customer, please reach out to the respective customer directly.  We are not responsible for the privacy or data security practices of our customers, which may differ from those set forth in this Notice.

If you are using this site as part of the workforce of an NRC Health customer or if you utilize other NRC Health online tools as part of your professional role of an NRC Health customer, this Notice does not apply to you or your information.  If you have questions regarding the privacy of information provided in such capacity, please reach out to your employer directly.
